Physicians CareConnection Uniting to Support Vulnerable Communities

In the heart of Central Ohio, individuals like Kiara Gholson face a myriad of challenges that extend far beyond their immediate health concerns. Navigating through the complexities of poverty, homelessness, and pregnancy, accessing quality healthcare can seem like an insurmountable task. In the midst of these difficulties, programs like Physicians CareConnection (PCC) emerge as comprehensive support systems that go beyond traditional medical care.

PCC recognizes that healthcare is not just about treating diseases; it's about addressing the underlying social determinants that significantly impact one's well-being. For many Central Ohioans, resources can be disconnected, access can be limited, and coordination of care may be lacking. Moreover, the affordability of healthcare remains a significant barrier for vulnerable populations. This is where PCC steps in with its unique model of patient-centered care coordination.

Kiara Gholson's journey with PCC is a testament to the transformative power of this program. Pregnant and homeless at the time, Kiara stumbled upon PCC through her insurance provider. “It's [PCC] had a significant impact and I have accomplished a lot,” Gholson remembered. “I learned a lot of life lessons.”
